Fluent output is not grounded output
An assistant can produce a confident response even when it has not been given the material needed to answer responsibly. In a business setting, the risk is not only factual error. It is an invisible shift from approved knowledge to plausible invention.
Source rules make the assistant’s scope visible. They identify what material is allowed, what should be cited or surfaced, what the assistant must refuse, and when a human must decide.
Build the source pack before the interface
A source pack can be small. It may contain a reviewed service description, a policy document, a product record, or a knowledge-base collection. What matters is that the material is named, current enough for the task, and structured so a reviewer can inspect why it was used.
Use review as an operating capability
Review is not a sign that the system failed. It is the mechanism that keeps important decisions accountable. Define the conditions that require review, the person who owns it, and how the assistant hands off context. This is the foundation for useful agentic work.
