Review protects the decision, not the tool

Automation is most valuable when it reduces routine effort while preserving accountability for consequential decisions. A review gate specifies when a human should inspect an output, what they should decide, and how that decision changes the next step.

Make escalation visible

Define the signals that trigger escalation: missing source context, an exception to policy, a high-impact action, or low confidence in a classification. The system should make these moments easier to see, not hide them behind a smooth interface.

Use review data to improve the workflow

Corrections reveal where instructions, source material, or handoffs are weak. Record the reason for a correction so the workflow can be improved through evidence rather than a cycle of undocumented edits.
